I. The conditions for assistance from Fuxing Palace for this case are:

(I) Identity conditions:

1.1. Residents or students experiencing personal emergencies (details of resident and student identity conditions are in the attached implementation plan): such as suffering from serious illness, natural disasters, or major accidents, leading to difficulties in their lives, studies, or medical care.
2. Sudden family economic changes: such as parents losing their jobs, divorce, or major accidents, leading to difficulties in life, studies, or medical care.
3. Requiring urgent assistance as assessed by the homeroom teacher: Students who are not low-income or middle-low-income households and do not receive proof of subsidy, but are genuinely poor due to their family situation and cannot afford tuition and lunch fees, and are assessed by the homeroom teacher as genuinely needing assistance.

(II) Application conditions and principles:

1.1. Applications must be submitted within six months of the emergency occurring, and assistance for the same item is generally provided once within six months of the event.

2.2. Those who have already received government or other unit subsidies in the current year must indicate this on the application form.

3.3. Supporting documents to be submitted with the application:

(1) Referral application form

(2) Photocopy of household registration transcript for the entire household within the last three months (with remarks column) or photocopy of household registration book.

(3) Proof of major accident, such as a copy of a medical diagnosis, death certificate, medical or funeral expense receipts, proof of major disaster, proof of unemployment, etc.
