Eligible students' parents with needs are requested to submit the following documents byMarch 14 (Friday)before:

1. A completed application form and a photocopy of the postal savings passbook of the student applying
2. A photocopy of the household registration transcript
3. A 2-inch photo (write the school name and student's name on the back, and clip it to the upper left corner of the application form with a paperclip)
4. A photocopy of the low-income or middle-low-income household certificate (if not available, please apply for the personal income statement of the student's parent or main caregiver for the most recent year at the National Tax Administration)

Submit to the registration office for verification and mailing.
