Parents are requested to complete the "2023 Taipei City Elementary School High Myopia Prevention Service Questionnaire" before November 20, 2023.

I. In accordance with the "2023 Taipei City Children and Adolescents Health Promotion and Health Education Service Plan" by the Taipei City Department of Health.

II. The Taipei City Department of Health (hereinafter referred to as "this Bureau") and this municipal government's Department of Education have subsidized students in grades 1-6 of elementary schools in this city with a professional vision check once a year since 2003, to help parents cultivate their children's regular vision checkups and understand myopia risks. To understand the daily eye-use habits of students in this city, this Bureau has commissioned National Yang Ming Chiao Tung University to analyze the aforementioned questionnaire as a reference for developing strategies related to childhood myopia prevention.

III. The online form for the aforementioned questionnaire (URL: https://forms.gle/pxocrnH3kFZSpDWk8) is attached. Parents who complete the questionnaire before the early deadline of November 20, 2023 (the deadline is November 30) will receive a "Schoolbag Reflective Rain Cover" from this Bureau through the school. Supplies are limited, so please encourage parents to fill it out.
