Love needs to be timely, eyes need to be regular - 113 Elementary School Student Professional Vision Examination Service

I. In accordance with the Taipei City Department of Health's "113 Taipei City Children and Adolescents Health Promotion and Health Education Service Plan".

II. The examination voucher referred to in this letter is the "Eye Protection Passport" or "Eye Protection Card". Since 111, it has been a unified version (URL: https://psee.io/5hagnd). The "Eye Protection Passport" is distributed to new students by schools after the start of the first grade of elementary school. The validity period is from the first grade to the sixth grade of elementary school, with one subsidy per year. If the "Eye Protection Passport" is lost, it will be reissued as an "Eye Protection Card", and its subsidy eligibility is the same as the "Eye Protection Passport".

III. The service notes for 113 are as follows:

(I) Subsidy recipients: Elementary school students in grades 1 to 6 who are studying or registered in Taipei City.

(II) Subsidy period: From January 1, 113, to December 31, 113 (students in the sixth grade of the 112 academic year will be subsidized until June 30, 113).

(III) Examination method: Present the "Eye Protection Passport" or "Eye Protection Card" issued by the Taipei City Department of Health to 165 contracted ophthalmology medical institutions (the list of institutions may change occasionally; the latest list can be found on the department's website/theme zone/children and adolescent health care/Taipei City High Myopia Prevention, URL: https://psee.io/5hagj2).

(IV) Subsidy content:

1. Professional vision examination items include: uncorrected visual acuity examination, best corrected visual acuity, refractometry before mydriasis, slit lamp examination, refractometry after mydriasis, and health education guidance.

2. The subsidy covers "professional vision examination fees" and "registration fees". If there is a need for subsequent medical treatment, parents should make separate appointments, which are not included in this subsidy.
