Lighting up new horizons for disadvantaged students: Subsidy program for myopia glasses

To inform about the "Lighting Up New Horizons for Disadvantaged Students" Eyeglasses Subsidy Program, a collaboration between the Yu-Yang Charity Foundation of Taipei City and the Department of Social Welfare of Taipei City Government.

I. In accordance with the letter from the Yu-Yang Charity Foundation of Taipei City, dated September 6, 113 (2024), Yu-Yang No. 1130090003.

II. The Yu-Yang Charity Foundation of Taipei City, understanding that students from impoverished families cannot afford eyeglasses due to their families' lack of financial capacity, which leads to academic lag or inability to concentrate in class due to poor vision, has specially organized the aforementioned subsidy program. Families unable to afford eyeglasses for their children can prepare the application materials and submit them to the foundation.

III. The application period for the first semester of the 113 academic year (2024-2025) for the aforementioned subsidy program ends on October 31, 113 (2024) (based on the postmark date).

IV. Application Eligibility and Notes:

(I)The subsidy primarily covers the cost of "eyeglasses" and does not include contact lenses..

(II) Please apply as aschool unit with a consolidated list. Individual applications are not accepted..

(III) Priority will be given to students who are referred by the foundation to partner optical shops. If you cannot visit a partner shop, please call the foundation to inform the review committee of the reason for further review. Contact number: 02-2711-4888.

(IV) Eligibility is primarily for students below high school (under 18 years old).

(V) Schools wishing to apply should prepare the required documents, as listed in Section V, items (I) to (IX), and mail the hard copy documents to the "Eye Protection Program Review Committee" of the foundation. Once the eligibility is approved, a dedicated person will contact the student's parents to assist with appointment scheduling for eyeglasses and the reimbursement process.

V. Required Application Documents:

(I) Submission list from the school or social welfare institution.

(II) Individual application form for each case. Each case requires a separate form, and schools must fill in the "Unit Opinion or Review Description" column.

(III)A copy of the new household registration transcript or a copy of the household registration transcript issued within one year (including all notes)..

(IV)Proof of low-income, low-middle-income status, or a certificate of financial hardship for the applicant.

(V) A copy of the cover of the savings passbook for the remittance account to receive the subsidy (the account must be in the student's name).

(VI) School vision screening report or relevant diagnostic information from an ophthalmologist or eye clinic indicating poor vision.vision screening results indicating poor vision.

(VII) Student appointment form for the eye protection program in cooperation with the foundation and University Optical Group. (This is for partner optical shops. Please fill out the appointment form and submit it with other documents to the foundation. If approved, a dedicated person will contact the student to schedule an appointment for eyeglasses).

(VIII) Personal data collection, processing, and utilization consent form (consent to use anonymized case stories on the official website for sharing and public credit).

(IX) Applicants must personally sign the foundation's receipt form when submitting the application.

(X) After the eyeglasses are made and the invoice is obtained, please mail the original invoice for the eyeglasses cost to the foundation. Funds will be disbursed after review and confirmation. Please also provide photos of the student's life and learning situation before and after getting the eyeglasses for review and record-keeping by the competent authorities.

VI. For related inquiries, please contact the Yu-Yang Charity Foundation of Taipei City (Phone: 02-2711-4888). Schools should compile the required application documents and mail them to: "Eye Protection Review Committee, 12F-1, No. 60, Section 4, Zhongxiao East Road, Da'an District, Taipei City 106070."

VII. After the foundation's review committee confirms the application documents and eligibility, they will conduct interviews and assessments. Approved applications will receive a subsidy amount at their discretion (not the full invoice amount). Submitted documents will not be returned, regardless of approval status.
