1. Please complete the"Registration Form - Attachment 1"(You can choose1to4options based on your preference).
2. Before the end of the workday onMay 9th (Friday), please submit the paper registration form to Teacher Yin Bichung, Academic Affairs Office(After the school registers online uniformly, the system will automatically sort the admission results).

Notes:

1. This activity is limited to fifth, fourth, and third-grade students who are registered residents of this city in the113academic year.
2. Please fill out the paper registration form neatly and clearly(The Academic Affairs Office needs to fill in each item online to complete the registration).
3. For children classified as categoryA, the registration form must be signed by the homeroom teacher before being submitted to the Academic Affairs Office.
