The registration for the 112 academic year's first-grade elementary school assessment and placement is from 11/28 to 12/9. Our school providesinclusive class distributed resource classspecial education services. If you need to register, please call the Special Education Section first (2936-0725ext.153) to prepare the registration information package, and pleasepick up the registration information package from the school's security office, or print and use the attachment of this message yourself.Pleasefill out the documents carefully and prepare the followingregistration materials, and submit them during the receiving hours (11/28-12/9, Monday to Friday, 9:00 AM to 4:00 PM daily). Late submissions will not be accepted.Due to entry control measures during the epidemic period,please make an appointment by phone with the Special Education Section in advance2936-0725ext.153to collect the materials (materials must be handed over in person), thank you for your cooperation!. I.According to Taipei City's112academic year's plan for the assessment and placement of students with disabilities entering elementary school.
II. The schedule for the aforementioned plan is as follows:
(1)Registration period: November 28, 111 to December 9, 111(Monday to Friday, 9:00 AM to 4:00 PM, registration is not accepted on holidays).(2)Educational assessment: November 28, 111 to February 5, 112.(3)Assessment and placement meeting: February 17, 112 to March 17, 112.III. RegistrationI. Eligibility: Children with disabilities who are of school age, have resided in Taipei City and are within the school district(not a resident by lodging, with actual residence), and are six full years old. Born before September 1, 106
(including September 1st).II. Registration method: Parents or guardians must register in person, or authorize another person to register with a letter of authorization (letter of authorization is attached).III. Registration time: November 28, 111 to December 9, 111(Please contact the Special Education Section at 29360725-153 Teacher Chen before registration for related matters)IV. Registration location: Shih Chien Elementary School Counseling Office (Children attending with parents should register at their parents' workplace).IV. Registration materialsI. Basic information to be attached(1)Registration form(If parents have joint custody, both parents must sign)(2)Checklist(Please check each item)(3) Original household registration transcript
(including detailed notes, original will be returned after verification) and a copy1copy(4) Affidavit of actual residence (fill in according to the version chosen by the parents for resource class or special education class).(5)Relevant supporting documents (one of the following documents is sufficient for registration)1.Disability certificate(if not expired for re-assessment) original (will be returned after verification) and a copy1copy.2.Comprehensive report of joint assessment for children from regional or higher-level hospitals
(if the next assessment date is after the registration date; if the comprehensive assessment report does not indicate the next assessment date, the report date must be within 1
year of the registration date).3.Diagnosis certificate from a regional or higher-level hospital within six months(if the diagnosis certificate date is within6 months before the registration date).II. Supplementary materials for special education needs assessment(1)According to each disability category, the following relevant materials should be provided:1.Students with hearing impairment must provide6months of audiology reports from regional or higher-level hospitals or hearing assessment reports from the Hearing Resource Center.
2.
Students with visual impairment must provide6months of vision diagnosis certificates from regional or higher-level hospitals or visual function assessment reports from the Vision Resource Center.
3.Students with physical frailty must provide diagnosis certificates that prove their physical frailty, need for long-term care, and affect learning activities.
4.
Students with autism must provide a diagnosis certificate from a child psychiatrist.
(2)If students have received special education services or early intervention services before school, please provide the following relevant materials (optional):1.Transition service data form for students with disabilities.2.
Individualized Education Plan for students with disabilities in the preschool stage for the111academic year.3.Summary of teaching (itinerant) counseling and assessment data within six months.
4.Preschool assistive device needs assessment data.5.Early intervention service data.

check the content of the materials item by item to ensure that the submitted materials are complete.身心障礙證明 (未逾重新鑑定日期者)正本(驗畢發還)及影本1份。
2.區域級以上醫院兒童發展聯合評估綜合報告書(下次鑑定日期在報名日期之後者;倘評估綜合報告書未註明下次鑑定日期,則以評估綜合報告書日期在報名日期前1年內者為有效)。
3.區域級以上醫院半年內診斷證明(診斷書開立日期在報名日期前6個月內者)。
二、特殊教育需求評估補充資料
(一)依各障礙類別,應檢附下列相關資料:
1.聽覺障礙學生須檢附6個月內區域級以上醫院評估聽力圖或聽資中心聽能評估報告。
2.視覺障礙學生須檢附6個月內區域級以上醫院視力診斷證明或視資中心視功能評估報告。
3.身體病弱學生須檢附可佐證其身體病弱,需長期療養且影響學習活動之診斷證明。
4.自閉症學生檢附兒童精神科專科醫生之診斷證明。
(二)學生倘於學前階段已接受特殊教育服務或早期療育,請檢附下列相關資料(可擇要提供):
1.身心障礙學生個案轉銜服務資料表。
2.學前階段身心障礙學生111學年度個別化教育計畫。
3.六個月內教學(巡迴)輔導及評估摘要資料。
4.學前輔具需求評估資料。
5.早期療育服務資料。
