2. Applications must be submitted online by4:00 PM on October 31, 111 (Monday). Please log in to the online system to register relevant information, and byOctober 31, 111 (Monday), 4:00 PMprintthe "Individual Application Form", "Proxy Authorization Letter", and "Requirements Form" (if there are no requirements, fill in "None" and sign). After signing, submit them to the school's registration office for application processing (Note: The applicant school must be the school in the household registration area).This will facilitate the subsequent school preliminary review meeting (parents must be present) and reporting to the Bureau of Education for review..

Notes:

1. When uploading relevant documents for online application, please note that if there are sections requiring signatures on the forms (checklist, application form, authorization letter, requirements form), you must upload scanned files of the signed documents.
2. Upload the signed checklist, application form, authorization letter, and requirements form, merged with the plan, to the non-school website application zone.
3. Upload a household registration transcript or household book (must be within the last 3 months, with no omitted notes, and must be registered in Taipei City!!!)
4. The list of teaching staff in the uploaded plan must be accompanied by academic qualifications.
5. Upload photos of the learning environment (mainly of your home).
6. For those who previously studied at a school, please attach a description of your school learning situation (description of the student's current situation). (Exempt if none)
