2. Relevant information must be logged into the online system before12:00 AM on April 30, 111, and before 4:00 PM on May 2, 111 (Monday), print the "Individual Application Form", "Proxy Authorization Letter", and "Demand Form" (if there is no Demand Form, fill in "None" and sign). 1. When uploading relevant documents for website application, please note that if there are sections requiring signatures on the forms (checklist, application form, authorization letter, demand form), scanned files with signatures must be uploaded.
2. The checklist, application form, authorization letter, and demand form, after being signed and merged with the plan, should be uploaded to the non-school website application zone.
3. Upload household registration transcript or household book (must be within the last 3 months, and remarks cannot be omitted)
4. The list of teaching staff in the uploaded plan must be accompanied by academic qualifications.
5. Upload photos of the learning environment (please focus on home)
6. For those who previously studied at school, please attach a description of the school learning situation (description of the student's current situation). (If none, no attachment is required)
