I. In accordance with the Taipei City Department of Health's "112 Taipei City Children and Adolescents Health Promotion and Health Education Service Project".
II. The following are the main points regarding the free professional vision examination provided by this service (announcement attached as Attachment 1):
(I) Eligibility for subsidy: Children enrolled in Taipei City elementary schools from grades 1 to 6 in 112, who hold a "Vision Protection Passport" or "Vision Protection Card" issued by the Taipei City Department of Health and have not yet received a free professional vision examination in the current (112) year.
(II) Subsidy period: From January 1, 112, to December 31, 112.
(III) Please use the Vision Protection Passport/Vision Protection Card that corresponds to your grade level for vision examination services. The usage period of the Vision Protection Passport and Vision Protection Card is not limited by the original printed grade level, academic year, or examination year. However, each person is limited to one professional vision examination provided by this department per year. Children who have not received this examination subsidy in the current (112) year can use a blank page of the "Vision Protection Record Form" in their Vision Protection Passport or Vision Protection Card to record their vision examination results (children in grade 6 in the second semester of the 111 academic year are subsidized until June 30, 112). If the aforementioned "Vision Protection Record Form" is insufficient, please apply to the Taipei City Department of Health for a blank "Vision Protection Record Form" for the children to use (for the version of the Vision Protection Passport/Vision Protection Card, please refer to Attachment 2).
(IV) Location of vision examination service: Designated ophthalmology contracted medical institutions for this service (https://bit.ly/高度近視合約院所).
III. Subsidy content: The subsidy for this service includes the registration fee (including co-payment) and the professional vision examination (uncorrected visual acuity test, best corrected visual acuity, pre-mydriatic refraction, slit lamp examination, post-mydriatic refraction) fees. If children have other medical needs after the examination, please schedule a separate appointment for medical treatment. If parents request related treatment on the same day, to avoid misunderstanding, hospitals have been instructed to clearly explain the scope of the subsidy, treatment methods, and related incidental costs, and to proceed with medical treatment under the National Health Insurance after parental consent.
