To encourage citizens to engage in volunteer service, the Ministry of Health and Welfare awards individuals who have accumulated 3,000 or more hours of service, possess a Volunteer Service Record Book, and hold a Volunteer Service Performance Certificate. Those who have completed 3,000, 5,000, or 8,000 hours of service will be awarded the "Bronze Medal," "Silver Medal," and "Gold Medal," respectively, along with a certificate. Each person is limited to receiving one medal of the same rank.

Please use the new (vertical) performance certificate for issuance. However, old (horizontal) certificates issued before August 3, 2021, are still valid. If a volunteer serves in multiple organizations, each organization should issue a separate performance certificate.

The service hours are calculated from January 22, 2001, to May 31, 2023. Service hours prior to this period will not be counted. Please list the start and end dates of service in detail. The issuance date should not be later than June 30, 2023.

Applicants should submit their documents to the Documentation Section of the Counseling Office by July 20.
