Taipei City 113 Academic Year Plan for the Identification and Placement of Students with Disabilities Entering Elementary School

    Taipei City 113th Academic Year Identification and Placement of Students with Disabilities for Elementary SchoolRegistrationThe registration period is from 11/27 to 12/8.Our school providesregular class distributed resource classesspecial education services.If you wish to register, please call the Special Education Section (2936-0725 ext. 153) in advance to inform usso that we can prepare the registration package. Please pick up the registration package from the school's security office, or print the attachment of this message yourself.

    Please fill out the documents completely and prepare the following registration materials, and submit them during the submission period (11/27-12/8Monday to Friday, 9:00 AM to 4:00 PM daily). Late submissions cannot be accepted..Parents are requested to make an appointment by phone with the Special Education Section at 2936-0725 ext. 153 for submission (materials must be handed over in person).Thank you for your cooperation! The relevant explanations are as follows:

I. In accordance with the Taipei City 113th Academic Year Identification and Placement Work Plan for Students with Disabilities Entering Elementary School.

II. The timeline for the identification plan is as follows:

(1) Registration: November 27, 2023 to December 8, 2023

(2) Educational Assessment: November 27, 2023 to January 5, 2024.

(3) Identification and Placement Meeting: February 16, 2024 to March 15, 2024.

III. Regist     ration

1. Eligibility for Registration:Students with disabilities who are registered residents of Taipei City and belong to the school's district (not residing temporarily and having actual residence),are of school age, and were born before September 1, 2018 (inclusive of September 1st).107year9month1before (inclusive of9month1st).

2. Registration Method: Legal representative (parents or guardian) to register in person, or authorize another person to register with a power of attorney (power of attorney is attached).

3. Registration Period:November 27, 2023 to December 8, 2023 (Please bring complete documents when registering.,Please call to arrange related matters before registration: 29360725-153, Ms. Chen, Special Education Section)

4. Registration Location: Shih Chien Elementary School Counseling Office (Students attending with parents register at their parents' workplace).

5. Registration Materials:

   I. Basic Information Attached

(1)Registration Form(If parents have joint custodyboth parents must sign)(2)Checklist(Please check item by item, parents)

(3) Original Household Registration Transcript(including detailed notes, original will be returned after verification)and a copy1copy(4) Affidavit of Actual Residence (fill in the version for resource class or special education class according to the parents' wishes).

1copy.2.Comprehensive Report of Children's Development Joint Assessment from a regional or higher-level hospital

(if the next assessment date is after the registration date; if the comprehensive assessment report does not indicate the next assessment date, then the assessment report date is within one year before the registration date).1yearvalid).3.

Medical Certificate from a regional or higher-level hospital within six months(if the diagnosis certificate was issued within 6 months before the registration date). 6months).II. Supplementary Information for Special Education Needs Assessment(1)

According to the disability category, the following relevant documents should be provided:1.Students with hearing impairments must provide an audiology report from a regional or higher-level hospital or an audiology assessment report from an audiology center within 6 months.6months.2.

Students with visual impairments must provide a vision diagnosis certificate from a regional or higher-level hospital or a visual function assessment report from a visual information center within 6 months.6months.3.

   Students with physical disabilities must provide a diagnosis certificate that proves their physical condition, requires long-term care, and affects learning activities.

4.Students with autism must provide a diagnosis certificate from a child psychiatrist.(2)If the student has received special education services or early intervention services before entering school, please provide the following relevant documents (optional):

1.Transition Service Information Form for Students with Disabilities.2.Individualized Education Program for Students with Disabilities in Preschool for the 112th Academic Year.

3.Summary of teaching (itinerant) guidance and assessment within six months.4.Preschool assistive device needs assessment information.

5.Early intervention service information.

lPlease read the "Taipei City 113th Academic Year Identification and Placement Work Plan for Students with Disabilities Entering Elementary School" carefully,

and follow the"Checklist of Required Documents for Registration of Students with Disabilities Entering Elementary School in the 113th Academic Year",and check the contents of the documents item by item to ensure that the submitted documents are complete.

2.學前階段身心障礙學生112學年度個別化教育計畫。

3.六個月內教學(巡迴)輔導及評估摘要資料。

4.學前輔具需求評估資料。

5.早期療育服務資料。

  請務必詳閱「臺北市113學年度身心障礙學生入學國民小學鑑定及安置工作計畫」,並按照「113學年度身心障礙學生入學國民小學鑑定及安置報名應備資料檢核表,逐項檢查資料內容,確定繳交資料完整。
