Parents are requested to complete the "2023 Taipei City Elementary School High Myopia Prevention Service Questionnaire" before November 20, 2023.

I. In accordance with the "2023 Taipei City Children and Adolescents Health Promotion and Health Education Service Plan" by the Taipei City Department of Health.

II. The Taipei City Department of Health (hereinafter referred to as "this Bureau") and this municipal government's Department of Education have subsidized students in grades 1-6 of elementary schools in this city with a professional vision examination once a year since 2003, to help parents cultivate their children's regular vision check-ups and understand myopia risks. To understand the daily eye-use habits of students in this city, this Bureau has commissioned National Yang Ming Chiao Tung University to analyze the aforementioned questionnaire, as a reference for developing strategies related to children's myopia prevention.

III. The online form for the aforementioned questionnaire (URL: https://forms.gle/pxocrnH3kFZSpDWk8) is attached. For those who complete the questionnaire before the deadline of November 20, 2023 (the deadline is November 30), this Bureau will present one "Schoolbag Reflective Rain Cover" through the school. The quantity is limited, so please encourage parents to fill it out.
