Registration Period: From February 19, 113 (Monday) to March 8 (Friday) (8:00 AM to 4:00 PM on working days).

Registration Materials:

(1) Completed Registration Data Checklist [Attachment 2]

(2) Please attach the following documents and proof of eligibility in order:

1. Joint Placement Registration Form [Attachment 3].

2. Educational Certificates: Applicants for the first year of junior high school should submit a copy of their elementary school graduation certificate or a certificate of enrollment; applicants for the kindergarten or first year of elementary school are exempt from submitting the aforementioned documents.

3. A copy of the household registration transcript or household registration certificate.

4. Supporting documents for students with disabilities (choose one to submit):

(1) For those with a valid disability certificate, please submit a copy of the front and back.

(2) For those with a certification (roster) from the county/city assessment committee or related documents for placement application with the county/city assessment committee, please submit a copy.

(3) For kindergarten and elementary school students, submit one original copy of the Integrated Report of the Joint Assessment of Child Development Centers issued within one year or with a valid period (the next assessment date is after the registration period), or a diagnostic certificate from a regional hospital or above issued within 6 months.

5. One 2-inch passport-style photo with head uncovered and facing forward, with the name written on the back and affixed to the Joint Placement Registration Form [Attachment 3].

6. One A4 registered self-addressed envelope with the recipient's name and detailed address written on it.
