The Bureau requests your school to distribute one "Eye Protection Passport" to each first-grade elementary school student at the beginning of the new academic year in September each year. The "Eye Protection Passport" is valid from the first to the sixth grade of elementary school, with one subsidy per year. If lost, an "Eye Protection Card" will be reissued, and the subsidy eligibility is the same as the "Eye Protection Passport".

III. The subsidy matters for 114 are as follows:

(I) Subsidy recipients: Elementary school students in the first to sixth grades who are studying or registered in Taipei City.

If further treatment is required based on the examination results, or if there are other medical needs, patients should seek medical attention through the National Health Insurance separately, which is not included in this subsidy.
