Eligible students' parents with needs are requested to submit the following documents byMarch 14 (Friday).

1. Completed application form and a photocopy of the student's postal savings passbook for the application
2. Photocopy of household registration
3. 2-inch photos (write the school name and student's name on the back, and clip them to the top left of the application form with a paperclip)
4. Photocopy of low-income or middle-low-income household certificate (if not available, please apply for the individual income statement of the student's parent or main caregiver for the most recent year at the National Taxation Bureau)

Submit to the registration office for verification and mailing.
